Collaborations With Other Services & Professionals

Staff work with various community committees and working groups to contribute to community well-being and to develop collaborative service responses to social problems that directly impact the mental health and functioning of children, youth and their families.Whenever possible the Centre contributes to the development of new initiatives to strengthen or develop services, offers leadership to the Huron Perth Children's Mental Health Network and supports community planning bodies relevant to services that benefit children, youth and families.

Consultations

Huron-Perth Centre staff offer consultations to schools, physicians, and other community professionals when guidance is needed to respond to an emerging need and/or when assistance to facilitate a referral is sought. With no client consent, general advice is given; with client consent, more specific guidance can be given. From time to time staff participate in community events to support public education on topics related to the expertise of Centre staff.

Membership on the Huron Perth Integrated Children's Services Review Team

A representative from the Centre serves on this inter-agency/inter-disciplinary team that is committed to review children's need for services, strengthen service collaboration and problem-solve situations for which a child/youth's unmet needs are compromising their ability to be successful in school. Discussions occur through client consent. Leadership comes from Huron-Perth Centre, Huron-Perth Children's Aid and personnel from both Avon Maitland District School Board and the Huron Perth Catholic District School Board with other community professionals free to attend as appropriate.

Membership on Community Services Coordination Network

A representation from the Huron-Perth Centre serves as a member of the Community Services Coordination Network's Local Resolution Team (LRT) along with other professionals committed to serve children and youth. The team performs two useful functions:

  • Service Resolution Process - with client consent this process provides a forum through which individuals, families and service providers explore a wide range of options that contribute to an individualized plan to address complex needs. Clients whose needs may be challenging, require higher levels of inter-agency coordination and are often deemed "hard to serve" are discussed in an effort to fully utilize all community-based options before accessing more intrusive and/or specialized regional resources.
  • Access to Residential Care - with client consent this process ensures that those individuals most in need receive assistance first by establishing one waiting list for regional residential services. LRT members meet to review client information pertinent to recommendations for residential care.
